Through Integration Supporting People To Be Well And Independent At Home

 

Presentation by Torbay and S Devon NHS Foundation Trust representatives:-

Ann Wagner – Director of Strategy and Improvement

Dawn Butler – Deputy Director

Lee Baxter – Assistant Director, Community Health and Social Care Services Southern

Minutes:

The Committee received a presentation by Torbay and S Devon NHS Foundation Trust representatives:-  Ann Wagner ( Director of Strategy and Improvement),  Dawn Butler (Deputy Director)  and Lee Baxter ( Assistant Director, Community Health and Social Care Services Southern) , on the integration of  the new care model, putting the individual at the centre, with Health and Well Being Centres to support people to be as well and independent as possible,  engaging with communities and working with care partners to stay health and socially active and for the prevention of ill health.  Alongside the centres were Clinical hubs at four community hospitals.

 

The shift in approach was to provide a reliable alternative to hospital admission, reducing the reliance on bed-based care and maintaining independence.

 

Information and services access had moved to one single point of contact for Torbay and one for South Devon, sharing records with partners and improved technology for mobile working in the community.

 

Indicator measures and data available to date showed very positive feedback with coastal services (Dawlish and Teignmouth) in particular working well and leading the way.   Indicators included:-

-Recent Peer Review;

-Changes, closures, investments and recruitment processes working well, with improved traffic light indicators January - June 2017 showing less bed occupancy but more able to support patients at home (all green services at Dawlish and Teignmouth) ;

-National leader for Integrated services with DCC, Torbay and the NHS working together as a single team;

-Delayed transfers of care (social care packages);

-Length of  stay in residential homes reduced ;

-Multi-Disciplinary Team getting patients out of hospital quicker with investment in managing patients at home.  

 

Discussion points raised by Members included:-

 

-Age related/socio demographics and encouraging a broader section of society to engage in health and wellbeing planning with public health teams eg relocating  services to more accessible site at Dartmouth;

 

-Further promotion of the new model’s success with emerging statistics;

 

-Demands on the residential sector and their role in the overall care picture;

 

-Observation of best practice eg Teignmouth Multi-Disciplinary Team meetings.

Local Member Updates

An opportunity for Members to update the Committee on community initiatives and responses to service changes in their own division.

Minutes:

The Chairman reported that following on from the discussion at Minute 20 above regarding the promotion of health and wellbeing he would welcome the opportunity for John Coates, Chairman of the South West Youth Games (formerly the Devon Youth Games) and Nikki Taylor, Teignbridge District Council organiser, to explain the Games and the involvement of Devon  Councils (with Teignbridge being a key promoter) in more detail,  to a future meeting of this Committee.

Around 2,000 young people (following extensive trials) had taken part in the 2017 South West Youth Games finals at Okehampton on Sunday 9 July.  The Games had been opened by former Olympic medallist , international Modern Pentathlete and Patron of the South West Youth Games Heather Fell, who commented It’s so important that young people have the opportunity to take part in events like the South West Youth Games.  For hundreds it can lead onto a lifetime of enjoyment and the health and social benefits of sport. For a few it might even lead to the Olympics or Paralympics. “For anyone who takes part, it is a fantastic journey to be part that builds new friendships and exciting new experiences.”  

Young People from every South West community had the opportunity to compete for their Devon district or the combined Cornwall team in over 20 sports, including able-bodied and disability disciplines for male and female competitors ranging from school year 3 to year 13.

RESOLVED that this item be added to to this Committee’s work programme.
